If you are not feeling well or are not comfortable coming into the office, please consider requesting a Telemedicine appointment for your follow up visits with your provider. Telemedicine appointments are not available for new patients, new injuries, and some post operative visits. Your provider or athletic trainer will let you know if a Telemedicine visit is right for you.

Telemedicine appointments are only available during regular business hours. Please remember that a request for an appointment is a request and not a scheduled appointment until confirmed by our office.
Telemedicine Appointment Instructions
Before the date of your appointment, please complete and sign the Telemedicine Consent Form and return the form to our office via email to apprequests@nationalsportsmed.com or fax to 703-729-5833, so we may bill your insurance carrier for your visit.
Prior to your appointment one of our Patient Care Coordinators will call you to collect your copay for the visit if applicable.
