Release of Medical Records

Authorization for use and/or disclosure of protected health information.

Important Information: Once the records are ready, you will receive an invoice from us via email.  Payment can be made over the phone or in the office. 

The cost for medical records is as follows:

  • $0.50 per page for the 1st 50 pages
  • $0.25 per page thereafter
  • $5.00 per Radiology CD

Once payment is collected, we can mail the records out, fax them to a location of your choosing, or you can collect them directly from the office.

Please note: Once your request has been processed (Downloaded and/or Printed) the invoice amount will be your responsibility. There is no charge for medical records sent to primary care physicians, referring physician or physicians that we refer you to, in order to facilitate the continuation of care.

Login into your Patient Portal to Request your Records

Login and visit our Message Center. Complete the “Request of Medical Records” and someone will be in contact with you shortly to coordinate your records. (Note: There is a 7-10 business day turn around time for all medical records.)
